Prof Health and Social Work Services

This Service Unit offers the professional guidance to complete the holistic service model. 

Health care activities are centralised around a home-based nursing care unit which, apart from doing assessments where the community resides, also provides primary health care to the community through a mobile clinic and sickbay. From here patients in need of palliative care are referred to the MES Impilo Care Centre. A holistic approach is taken when addressing the HIV/AIDS pandemic and each patient is empowered to take responsibility for their health through the provision of different services at the Wellness Clinic. This ultimately leads to a sustainable health status. Educational campaigns and support groups empower the community to make positive choices regarding their health. (At present this service is only applicable to Johannesburg.)

 
Professional social work services within MES cover a wide variety of activities. Once a client has gone through the intake phase, thorough assessment is done by a multi-disciplinary team, after which a Care Plan is established in conjunction with the client. An Individual Development Plan (IDP) is then drawn up to ensure the implementation of the Care Plan. From the assessment clients are referred to different services within the organisation as part of the intervention phase of the model. Statutory social work services are provided to families and individuals. These mainly focus on foster care, family preservation and reunifi cation, interventions for abused women and children, child neglect and abandonment and the guardianship programme for youth in conflict with the law. Monitoring of clients’ IDPs and relevant interventions promotes a sustainable exit from the programme.
